A 38 year old female was previously diagnosed with Grave’s Disease and treated by a bilateral subtotal thyroidectomy two years ago in another state.  Routine postoperative bloodwork and urinalysis tests revealed no remarkable findings and a physical exam one year ago revealed some weight gain and slightly slower heart rate. 

The patient came back for a two year follow up and presented with edema in the extremities, fatigue, and sensitivity to cold.  Anorexia was reported, despite continued weight gain and the skin was noted to be dry and flaky.  Bradycardia was also noted and the rest of the thoracic auscultation was within normal limits.

Diagnostic tests performed included a repeat CBC with serum chemistries,  radioimmunassay and TSH levels.  Blood work confirmed low T3 and T4 hormones

and TSH levels and serum cholesterol was also slightly elevated.

Assessment: Hypothyroidism

Treatment included a gradual thyroid replacement with levothyroxine and a diet change to one with high fiber and low calorie was recommended.  Follow up serum thyroid levels will be done in two months.  Any adjustments in thyroid medication will be made at that time.
