Admission Diagnosis:             Bilateral Cataracts

History:                      

79 year old female noted a progressive gradual blurring of her vision and the appearance of halos around lights.  Ophthalmoscopy examination confirmed senile cataract development with visual acuity of 20/80.  Medical history is otherwise unremarkable. Patient is on arthritis medication and thyroid medication.

Plan:

Outpatient surgical extraction of the opaque lens and postoperative correction of visual deficits.

Course in Hospital:

On August 25, Mrs. C underwent uneventful phacoemulsification for extracapsular cataract extraction with the insertion of a posterior chamber intraocular lens.

The patient was discharged the next day with a combination antibiotic-steroid eye drops.

Discharge Diagnosis:              Senile Cataracts, OU

Operation performed:              Phacoemulsification

                                                Extracapsular cataract Extraction

                                                Posterior Chamber Intraocular lens insertion

 

Postoperative:

Mrs. C experienced improved distance vision with no loss of vitreous.

The patient was to remain on restricted activity with eye protection until the next checkup.

At the monthly checkup, no pain, hyphema, or iris prolapse were noted.

Corrective reading lens were prescribed for near vision 2 months postoperatively.
