____I understand that science courses require daily dedicated time for study and review in order to succeed in the course and I have made physical and real time in my daily/weekly schedule to successfully complete all the requirements for both the lecture and the lab courses.

____ I agree to seek assistance from my instructor or contact the ACCESS office for available tutors when having difficulty with the course material.

____I will have reliable internet access at all times (24/7) and that my computer, peripheral equipment, and current software and updated browsers are being maintained in good to excellent working order.

____I agree to take personal responsibility for any and all assignments that I turn in to my instructor; for all assignment due dates and times; and that turning in late work for any reason will result in point deductions and a possible zero (0) grade per syllabi/addenda for lecture and lab.

____I have read and understood the course requirements as stated in the lecture syllabus, lab syllabus, lecture addendum, lab addendum, lecture objectives, lab objectives, and additional course requirements including but not limited to the textbook, computer, & internet access.
